NEW MEMBERSHIP APPLICATION<\/h4>\n

  1. Completed membership\u00a0application \/ indemnity form.<\/li>\n \t
  2. Proof you are a resident or rate payer of the Mornington Peninsula Shire. (drivers licence, rates notice, utility bill, etc)<\/li>\n \t
  3. Include an email address and contact phone number.<\/li>\n \t
  4. Proof of being the parent or the legal guardian if joining children under 18 years.<\/li>\n<\/ol>\n

    PARENTS<\/h4>\n
      \n \t
    1. The father and mother of the child<\/li>\n \t
    2. The spouse of the father or mother of the child<\/li>\n \t
    3. The domestic partner of the father or mother of the child<\/li>\n \t
    4. Any person who has parental responsibility for the child.<\/li>\n<\/ol>\n
